The Release / Authorization Form is required of all travel team players. Intramural players do not use it. This form MUST be notarized and must be notarized within the last 365 days. This is a link on the EPYSA web page. The online form must be completed at least 30 days before your team plays in an out of state tournament. This applies ONLY to our teams registered as EPYSA and is not applicable to teams registered with US Club. (requires Adobe Acrobat Reader)

This form must be printed, filled out and returned to the club by any person who will be receiving compensation from the club for coaching or administrative help. This form must only be submitted once, NOT yearly.

Travel teams need to submit this form along with any payments due to coaches or clinicians. Forms should be received by LMSC no later than the 20th of the month to get paid for that month.
